For the past six years, Unite Us has been connecting healthcare entities with social service providers to improve overall care to consumers, showing the importance of social care, along with clinical care, as part of a quality life. The company has announced they received $35 million in Series B funding that will be used to grow existing networks and empower society to strive for better living standards.
While an important aspect of health is medicine, the systems within Unite Us work to ensure that people also have access to basic needs like food, housing, and transportation. Health systems, health plans, employers, and government all play a vital role in combining forces to create a curated community goal, but it has always been a challenge finding and maintaining efficient partnerships.
Unite Us continually tracks individual outcomes within each community to determining where health and quality of life are improving and how costs can be dramatically reduced. Their technology platform and networks offer powerful data that community members and health care organizations can use to impact data models, create predictive analytics, and fully integrate the services across the country.
